The breakdown field of the center conductor should be multiplied by a (up to values of 1.2) for better accuracy in cases where the field at the center conductor is highly enhanced. 

The "mean" field is just the field between parallel plates with the same distance between conductors and voltage.

In cases where the entire area is not stressed, use that area which exceeds 90% of the maximum stress.
